Nursing Facility Transition

We assist in helping individuals to live in the community and transition them to assisted living care homes when possible. The goal is to facilitate nursing facility transition back to a home like setting and or prevent skilled nursing admissions for residents on long term support services (LTSS) and for individuals at risk of premature institutionalization. Our mission is to help individuals who are transitioning from a licensed health care facility to a living arrangement in a Residential Care Facility for Elderly (RCFE) and Adult Residential Facilities (ARF).

This includes 24-hour care to assist with needs. The Assisted Living provider will be responsible for making sure that all of the individuals needs are met including assistance with activities of daily living (ADL’s) (IADL’s), meals, transportation, medication assistance, recreational activities, etc.

For Transition

To be eligible for Nursing Facility Transition Services, a Member:

  1. Has resided in a nursing facility for 60 days or longer; 

  2. Is willing to live in an assisted living setting as an alternative to a nursing home; and 

  3. Is able to reside safely in an assisted living facility with appropriate and cost-effective supports.

Nursing Facility Diversion

To be eligible for Nursing Facility Diversion Services, a Member must be: 

  1. Interested in remaining in the community; 

  2. Willing and able to reside safely in an assisted living facility with appropriate and cost-effective supports and services; and 

  3. Currently receiving medically necessary nursing facility level of care or meet the minimum criteria to receive nursing home services and in lieu of going into a facility, is choosing to remain in the community and continue to receive medically necessary services at an assisted living facility.

Our goal is to promote dignity, independence, and enhanced quality of life for those we serve." Enhanced Care Management is a new statewide benefit with Medi-Cal (CalAIM) to provide a personalized approach to care that addresses the clinical and non clinical medical needs of Medi-Cal beneficiaries enrolled in Medi-Cal managed care. We will assign a Care Advisor to each beneficiary who will strategize care and care services including health, wellness & senior housing.
